We will take the required time to properly diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan that provides for your optimal dental health. With your cooperation, we will complete a thorough comprehensive dental evaluation.

Patient Registration

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Note to Patient: Upon filling out your first name, last name, email and at least one phone number field (a valid home or cell phone) a button will appear in the upper right corner titled “Save and Exit” . This button allows you to save and exit your form to complete at a later date. The system will send a registration email to the email provided on the form, so that you may setup a login to go back and finalize your registration form online.

Financial Policy

Patient Billing

For your convenience we accept Visa, MasterCard, Discover, American Express, Cash and Care Credit. We do not accept personal Checks. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your dental insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. 

Insurance Information

Our office accepts most PPO insurance policies. Your benefits for dental services depend on the arrangement between you, your employer, and the insurance company your employer is contracted with. We will bill the insurance company for services rendered, as a courtesy to you.
